Budget Worksheet

· ALL fields must be filled in before submission

· The AEL number can be found at www.rkb.mipt.org

· When submitting a budget always send it to Kathy Hedrick@adem.arkansas.gov as an attachment on an email

· Kathy will respond to your request by email

· All new items on the budget should be highlighted YELLOW

· All items to be replaced should be highlighted RED

· Keep a copy of the budget on you computer for future revisions
